While there may be certain scenarios that require a tester to perform more rigorous testing, supply more documentation and / or supplemental materials, and maybe even put on their sleuthing cap to try and narrow down the cause, for the most part a tester should be documenting exactly WHAT happens rather than speculate WHY it is happening.
